Fillable ACK and JURAT

Somebody had these CA forms available where you could fill in the info on your computer as well as "drop-downs" to use depending on if the form called for an individual or multiple signers. They really looked professional. I'm not that up on Word or Excel to make my own. Anyone have them available?
